The AHRQ-commissioned meta-analysis found moderate evidence for small-to-moderate improvements in anxiety, depression and pain, low or insufficient evidence for most other outcomes, and no evidence that meditation programs beat any active treatment such as exercise or CBT (goyal-2014). The positive update (goldberg-2022) still shows comparability with established therapies, not superiority — real, bounded effects for defined outcomes.

A mixed-methods study documented 59 categories of meditation-related challenges — anxiety, dissociation, trauma re-experiencing, perceptual changes, some with lasting functional impairment — including in practitioners with sound instruction and no psychiatric history (lindahl-2017). Systematic review puts adverse-event prevalence around 8 percent, comparable to psychotherapy generally (farias-2020).

Both major practice families work with thoughts, not against them: focused attention trains noticing distraction and returning; open monitoring trains non-reactive observation of whatever arises (lutz-2008). The founding secular instruction is attention with a non-judgmental stance, not thought suppression (kabat-zinn-1990) — and deliberate suppression tends to backfire (Wegner and colleagues 1987, Journal of Personality and Social Psychology, the white-bear experiments). The satipatthana instruction is to know the mind's states as they are (satipatthana-sutta, analayo-2003).

Two combined randomised controlled trials with active controls and much larger samples found no structural brain changes after MBSR — published from the same research community, by the Davidson lab (kral-2022). The broader imaging literature's methodological weaknesses were already flagged in tang-2015. Functional and behavioural effects remain under study; the short-course structural claim should be retired.

The bare attention reading of sati dates to twentieth-century reform Buddhism (nyanaponika-1962); canonical sati includes recollection, clear comprehension and an ethical path context (satipatthana-sutta, analayo-2003); mass lay meditation itself is largely a modern development (mcmahan-2008). Sharf (sharf-2015) states the strong form of the critique; Kabat-Zinn (kabat-zinn-2011) concedes transformation while defending recontextualisation. Continuity is contested — identity is not.
